The Caregiver contributes to the Adult Family Home (AFH) by providing care in accordance with the individualized care plan outlined for each member, electronic charting, administering prescribed medications, maintain physical environment cleanliness, prepare and serve nutritional meals, welcome families, medical personnel, and guests, assisting with daily living activities, keeping in compliance with Chapter 88 WI State Regulations and applicable regulations local, state, and federal laws, and supporting strategies related to the local day-to-day operations of Lambs Creek AFH.
QUALIFICATIONS
U.S. Work Permit and/or Social Security Card.
Must be at least 18 years of age.
High School Diploma or GED.
Community based residential facility (CBRF) training or CNA license preferred.
Current CPR / First Aid Certification (Online course offered in orientation to fulfill qualification).
PHYSICAL DEMANDS
The physical demands described here are representatives of those that must be met by an employee to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities/ailments/limitations to perform the essential functions.
While performing the duties of this job, the employee is regularly required to utilize all senses such as having visual attention, ability to talk to and listen for members, use smell for health and cleanliness, and hand dexterity with feeling in hands and limbs.
The employee is required to stand and walk. The employee must occasionally move up to 50 pounds, lifting 10 to 25 pounds constantly.
Lifting and transferring safely with use of equipment and/or body weight to help individuals get in and out of bed, wheelchairs, or other locations can be physically demanding, especially for those with limited mobility.
The employee must reach below and above shoulder height frequently and kneel, stoop and squat occasionally.
Stand 6 out of 8 hours.
WORK ACTIVITIES
Administer basic health care or medical treatments and prescribed medications.
Electronically document member health and maintain daily records of services, behaviors, or concerns.
Provide counsel, comfort, or encouragement to individuals or families.
Prepare and cook nutritious home-cooked meals per posted menu and provided recipes.
Drive vehicles to transport members utilizing company vehicles.
Perform housekeeping duties maintaining cleanliness standards with provided cleaners and tools.
Assist individuals with special needs in all daily tasks both physical and social/emotional.

RESPONSIBILITIES
Administer bedside or personal care, such as ambulation, range of motion, or personal hygiene assistance, including bathing, dressing, grooming, toileting, oral hygiene, incontinent care, transferring and getting to and from activities and meal modifications according to the individualized care plan.
Understands and ensures that individualized care plans are followed and ensures that specific information, changes, and observations of the members’ needs and preferences are electronically documented and relevant information communicated directly to administration.
Prepare and maintain records of member progress and services performed, reporting changes in member condition to administration.
Perform healthcare-related tasks, such as monitoring vital signs and medication, under the direction of registered nurses or physiotherapists.
Provide care that maintains each member's skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent members, turning, repositioning immobile members, and applying moisturizers to fragile skin, if indicated in the individualized care plan.
Anticipate the needs of the members based on previous experience, communication, and services provided.
Protect the personal belongings of each member, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc., and promptly report missing items according to established policy.
Communicate and interact effectively and tactfully with the members, visitors, families, peers, and supervisors.
Greet residents, visitors, and staff courteously and in a professional manner that respects the dignity and confidentiality standards and promotes individual rights.
Perform housekeeping duties, such as cooking, cleaning, and washing clothes or dishes.
Instruct or advise members on issues such as household cleanliness, utilities, hygiene, nutrition, or care.
Prepare and serve nutritious meals following the menu and provided recipes.
Transport members to locations outside the home, such as to physicians' offices or on outings, using a company provided motor vehicle.
Provide direct member care while promoting member dignity, independence, and choice, and by regularly observing member's physical and mental condition and directly reporting to House Manager or Managing Owner.
Follow standard precautions and safe work practices, established fire/safety/disaster plans, risk management, and security; reporting and/or correcting unsafe working conditions, equipment repair and maintenance needs.
Other duties as assigned.
