Lambs Creek is an owner-occupied care facility housing four (4) co-ed adults with developmental disabilities and/or physical disabilities in Dunn County Wisconsin. Our younger and cognitive population creates a relaxed and engaging environment for both employees and residents.

The Caregiver contributes to the Adult Family Home (AFH) by providing care in accordance with the individualized care plan outlined for each member, electronic charting, administering prescribed medications, maintain physical environment cleanliness, prepare and serve nutritional meals,  welcome families, medical personnel, and guests, assisting with daily living activities, keeping in compliance with Chapter 88 WI State Regulations and related requirements for the day-to-day operations of Lambs Creek AFH. 

We provide nurturing, and individualized care for our members while maintaining self-respect, privacy, and personal dignity, as we provide for their daily physical, social, and emotional needs through engagement and wellness opportunities in home and in the surrounding community.
